- Prior to your appointment, please look over the COVID-19 screening questions and notify the office of your answers.
- When You Arrive: Please stay in your vehicle

- Mask Required: Please wear a mask before entering the building
No visitors will be allowed in the waiting room or exam room with patients unless needed for assistance. Come by yourself and have your visitor wait in your car.
Children who are patients are to be brought to the office with One Adult only.
